WRAPAROUND SUCCESS! Funding from The London Community Foundation enables Community Services Coordination Network to create a valuable educational tool.
For some adults with disabilities and for some families with children and youth who have complex needs, support services are required from more than one service provider. Managing this is not always an easy task particularly if you’re not from this area or from another country altogether.
Through the WrapAround initiative at Community Services Coordination Network (CSCN), families or individuals are matched with a personal facilitator and together develop family centered, individualized plans of care that connect the family members to other informal and formal social networks in the community. This process aims to give people the confidence and knowledge to be in charge of their own support services.
Through funding acquired from The London Community Foundation, an educational video was created specific to the WrapAround process and to the barriers to accessing services encountered by individuals and families of different ethno-racial communities. The value of this educational video cannot be underestimated as CSCN moves forward in their efforts to foster awareness, knowledge, understanding and skills in the area of cultural diversity and organizational cultural competence specific to their programs and services.
